A science graduate with an MBA in Management, Dr. Sivakumaran went on to complete his M.Phil and PhD in Management—all while being employed full-time. His graduation was completed before entering the workforce, but every subsequent academic milestone was pursued alongside demanding professional responsibilities. This unique balance enabled him to directly relate theory to practice and understand managerial logic with clarity and depth.

This dual journey allowed him to practice what he studied in real time. Concepts were not confined to textbooks; they were tested, refined, and validated through lived experience. His early years in the textile industry further strengthened this foundation. Trained by gurus who emphasized endurance, discipline, and execution, he learned to overcome fatigue, manage large workloads, and deliver results irrespective of volume or working hours. Long days and late nights became part of the process, preparing him for the scale, intensity, and unforgiving timelines of healthcare operations.

Dr. Sivakumaran entered the healthcare industry almost by accident. After spending over a decade in textiles, he sought a change. Overcoming the Odds: Challenges of the Early Years

In the 1990s, India’s healthcare system was still underdeveloped, with low public priority and limited institutional support. Hospitals faced immense challenges—no access to bank funding, interest rates as high as 24%, lack of standardization, manual billing and documentation, and a shortage of trained healthcare professionals. Despite these hurdles, the Chairman, an NRI doctor, established the hospital and overcame each obstacle step by step.

As the hospital demonstrated success, banks gradually extended funding at lower interest rates. KMCH adopted accreditation standards to uphold quality and introduced computerized billing at a time when such systems were rare. Transparent and ethical practices steadily earned patient trust.

To address manpower challenges, KMCH established multiple paramedical and allied health education programs, along with degree courses in science, commerce, management, and medicine. These institutions not only strengthened the healthcare ecosystem but also ensured a sustainable pipeline of trained professionals.

The Challenges Ahead

Hiring and retaining trained manpower remains a pressing challenge. While KMCH proudly has over 750 employees with more than a decade of loyalty, attrition among younger professionals remains high. The hospital invests significantly in training, accommodation, food, transport, medical coverage, and structured career progression to retain talent.

Rising operational and capital costs, rapidly evolving technology, escalating patient expectations, and increasing capex continue to shape a complex landscape. Yet, KMCH consistently balances affordability with excellence.

One of the most transformative innovations has been in energy management. With monthly electricity costs nearing ₹1.75 crores, reducing operational expenses became critical. After extensive brainstorming, KMCH invested in alternative energy sources. A 10 MW solar park spread across 40 acres now meets 60% of energy needs, while wind energy contributes another 20%.

The cost difference is substantial—solar energy at ₹3.50 per unit compared to ₹15 from the grid. The hospital is also exploring additional wind energy systems to generate power during nighttime hours. These initiatives have not only reduced costs but also enabled the conversion of laundry and kitchen operations from gas to electric energy, optimizing resources and improving efficiency.

Leadership in Practice

As an administrator, Dr. Sivakumaran’s primary responsibility is to ensure patient-centric care aligned with established guidelines. While doctors and paramedical staff handle direct patient care, the administrator’s role is to create an environment where clinical teams can perform comfortably and efficiently—ensuring patient satisfaction.

“Leadership in healthcare,” he says, “is not driven by a single philosophy but by a dynamic blend of democratic, inspirational, and collaborative styles—providing agility rather than rigidity. This flexible approach fosters trust, inclusivity, shared purpose, participatory decision-making, and a culture of continuous growth and accountability.”

Balance Beyond the Boardroom

“Maintaining a healthy work–life balance is essential, even in a 24×7 hospital environment,” shares Dr. Sivakumaran. Clearly defined working hours, emotional boundaries, and stress management are critical. Work stress is not carried home, and personal stress is kept away from work. Practices such as yoga, meditation, pranayama, exercise, and hobbies help rejuvenate the mind and body.

Delegating responsibilities—except in critical matters—creates time balance and trust. A supportive team culture and periodic vacations help prevent burnout, improve well-being, and ultimately enhance productivity and performance.

Building the Future: Expansion and Innovation

To address outpatient congestion, KMCH is developing a 1.75 lakh sq. ft. facility, with each floor dedicated to a specialty and equipped with pharmacy, phlebotomy, and billing counters. Pneumatic systems will transport samples to central labs, and a modular OT complex with six state-of-the-art theatres and a GI suite is slated for completion by 2026.

Expansion plans include a 350-bed super-specialty hospital in Chennai and a 300-bed exclusive children’s hospital, the first of its kind in the city. Technological advancements continue with the introduction of an AI-enabled Ethos Linear Accelerator, plans for a PET MRI (first in the state), and leadership in multi-organ transplant programs—lung, liver, pancreas, kidney, bone marrow, and heart. A skin bank for transplant procedures is also on the horizon.

A Vision Extending Beyond Borders

“Over the next two years, we plan to add 100 beds to our existing facility, complete the Chennai hospital, and achieve 75% completion of the Children’s Hospital,” assures Dr. Sivakumaran. AI-based applications will enhance the patient journey, while operational efficiencies will reduce costs. Advanced medical equipment will bring the latest technology into patient care.

“Our medical college will welcome its first batch of postgraduate students, and beyond NABH, we aim to secure JCI accreditation to meet international standards. With an increasing focus on medical tourism, we are confident KMCH will emerge as a leader in this space as well.”
